How to get on our waitlist

Four steps from application to waitlist placement -- here's exactly what to expect.

Get the Application Packet

Download the rental application using the button below, pick one up at our office at 105 E. Walnut Street, or call 269-388-3011 and we will mail or email it to you.

The application includes the rental application, the Social Security disclosure form, and the HUD supplemental contact form. The Resident Selection Plan is available as a separate download below.

Complete All Sections in Full

Every field must be completed -- write N/A for sections that do not apply. All adult household members must also sign where required.

Incomplete applications will not be added to the waitlist. If information is missing, you have 10 business days to respond before the application is returned.

Gather Your Required Documents

Return your completed application with: Government-issued photo ID, Social Security card, Birth certificate or proof of age, and Disability verification if applying under Second or Third Priority.

Submit Your Application

Applications may be submitted in person, by mail, or electronically. Electronic submissions must be encrypted and password protected.

Once received, we will review your application and run preliminary screening. If eligible, your name will be added to the waitlist by date and time received.
Social Security Number Requirement

Federal law requires all household members who are U.S. citizens, U.S. nationals, or eligible noncitizens to disclose and document their Social Security Number (SSN) as a condition of receiving HUD-assisted housing. You must provide a Social Security card or other acceptable documentation for every qualifying household member.

If a household member does not currently have an SSN, they must apply for one using Form SS-5 through the Social Security Administration before a unit can be assigned. Contact the SSA at 1-800-772-1213 (TTY: 1-800-325-0778) or visit ssa.gov.

Noncitizens who are not eligible for an SSN are not required to provide one, but must provide documentation of their ineligible immigration status.

Special provision: If you or any household member turned 62 before January 31, 2010 and do not have an SSN, please contact our office -- an alternative process may be available.
